Intake Coordinator - Home Health & Hospice

Signature Healthcare at Home is seeking a dynamic and exceptional Intake Coordinator to support our mission of providing life changing service to our Home Health & Hospice patients. The Intake Coordinator is an exciting opportunity to support our established and amazing Home Health & Hospice team! The ideal candidate is professional, organized, home health and hospice savvy, compassionate, experienced working with a team, driven, proactive, quality focused, creative, fun (!), technologically adept, positive, and overall awesome. This position is Full-Time and benefited.

The Intake Coordinator will be responsible for:

All aspects of the patient intake process

Establishes and maintains positive working relationships with current and potential referral sources

Ensures compliance with all State and Federal referral/intake regulatory requirements

Ensures seamless transition of patients to home care

Builds and monitors community relationships

Potential for cross-training in medical records and scheduling
