About the role

Signature Healthcare at Home Hospice is seeking a compassionate and experienced Registered Nurse (RN) to join our team as a PRN Weekend Admissions Nurse serving the Portland metro area. This position plays a critical role in supporting patients and families during their transition to hospice services by providing timely admissions, education, and coordination of care.

About the Role

The Weekend Admissions RN is responsible for responding to hospice referrals, completing admission assessments, developing initial plans of care, and ensuring patients and families receive the education and support needed upon entry into hospice services. This position works closely with physicians, referral partners, hospice leadership, and the interdisciplinary team to create a seamless admission experience.

Key Responsibilities

  • Respond to hospice referrals and admission requests during weekend coverage hours
  • Complete comprehensive hospice admission assessments in patients' homes, assisted living communities, skilled nursing facilities, or other care settings
  • Evaluate patient eligibility for hospice services in accordance with Medicare and regulatory guidelines
  • Coordinate admission paperwork, consents, and required clinical documentation
  • Develop and initiate individualized plans of care
  • Provide education regarding hospice philosophy, services, medications, symptom management, and available resources
  • Collaborate with physicians, nurses, social workers, chaplains, aides, and other interdisciplinary team members
  • Communicate effectively with referral sources, hospitals, and community partners
  • Ensure timely and accurate completion of documentation within the electronic medical record
  • Support patients and families with compassion, dignity, and professionalism during challenging transitions

Qualifications

Required:

  • Current Oregon Registered Nurse (RN) license in good standing
  • Minimum one (1) year of nursing experience
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • Strong assessment, communication, and organizational skills
  • Ability to work independently and efficiently in the field

Preferred:

  • Hospice experience
  • Home Health experience
  • Admission or intake experience
  • Knowledge of Medicare hospice regulations
